Consent for your child's nasal flu vaccine

The Burren Pharmacy is visiting local primary schools this autumn to offer the children's nasal flu vaccine. If you would like your child to receive it, you complete this form once, at home, before the clinic.

How it works

  1. Your school sends you a linkEach school has its own, so your child's school and class are already filled in.
  2. You complete the form at homeIt asks for your child's details, your contact details, and a short set of health questions.
  3. The school tells you the dateClinic dates are arranged between the school and the pharmacy.
  4. We visit the schoolA pharmacist gives the vaccine on site, with school staff present.
